Walk-in pre-participation exams for student athletes — NJSIAA forms completed same visit. Get your young athlete cleared to play in under 30 minutes.
Whether your child is signing up for fall soccer, joining the high school basketball team, going to summer camp, or trying out for a college sport, they'll need a current sports physical (also called a pre-participation exam, or PPE) before they can play. New Jersey schools and most camps require a physical signed within the past 365 days.
Sage Urgent Care offers walk-in sports physicals seven days a week. No appointment, no waiting weeks for your pediatrician's office to have an opening. Most exams take 20–30 minutes, and you'll leave with all your forms signed and ready to submit.
The pre-participation exam is designed to identify any condition that might affect athletic safety:
Evenings and weekends too. Come in after school, after practice, or on a Sunday afternoon — no missed work or class.
Bring your school's paperwork — we complete and sign it before you leave, no follow-up needed.
Lower than most copays for a pediatrician visit. We accept most major insurance plans.
Our staff is comfortable with kids of all ages. We explain everything we're doing and answer questions.
If your athlete has a recent injury that needs imaging before clearance, we can do it the same visit.
With your permission, we forward the exam to your child's pediatrician for their records.
Self-pay sports physicals at Sage Urgent Care are $50, which is well below what most insurance copays would be for a wellness visit. We can also bill insurance — most plans cover an annual physical, including a sports physical, with no out-of-pocket cost.
No — sports physicals are walk-in, seven days a week. We recommend coming in mid-morning or early afternoon when wait times are typically shortest, especially in July and August before fall sports start.
We complete all standard New Jersey school athletic forms (NJSIAA), camp medical forms, college sports clearance forms, and physical activity readiness questionnaires. Bring whatever forms your school, camp, or league requires — we'll fill them out before you leave.
Most sports physicals take 20–30 minutes. We'll review medical history, take vital signs, perform a physical exam, complete any required forms, and answer questions about training, injury prevention, or specific concerns.
A sports physical (also called a pre-participation exam) focuses specifically on whether it's safe for an athlete to participate in their sport. It covers cardiovascular health, musculoskeletal evaluation, and screening for conditions that could be aggravated by exertion. An annual physical is more comprehensive and covers preventive care, vaccinations, and overall wellness. Some kids need both — your school will tell you what's required.
Most kids with managed conditions (asthma, controlled epilepsy, ADHD, prior concussions, glasses, hearing aids) are cleared to play. Bring documentation of any condition and current medications. If we have concerns, we'll discuss them with you and your child's primary care doctor before signing off.
Bring (1) any forms your school or league requires, (2) a list of current medications and allergies, (3) records of any chronic conditions or recent injuries, (4) glasses or contacts if they wear them, and (5) their inhaler if they have asthma. A parent or guardian must accompany any child under 18.
Yes. We complete NCAA, club sports, intramural, and college pre-participation forms. Many colleges have specific paperwork — bring whatever your school sent, and we'll work through it together.
Bring your school's forms and walk in any day from 8 AM to 8 PM. Most exams are done in 20–30 minutes.
